Tirzepatide is a synthetic peptide built from 39 amino acid residues. Its backbone derives from the native glucose-dependent insulinotropic polypeptide sequence, altered at several positions to resist enzymatic cleavage. A fatty diacid group attached through a linker extends plasma residence time by promoting reversible binding to serum albumin. The molecule carries a net negative charge near physiological pH and has a reported molecular weight close to 4813 daltons. These features separate it from shorter incretin analogs and account for its prolonged dosing interval.
Pharmacologically, tirzepatide activates two distinct G protein-coupled receptors: the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor. Binding at each target triggers cyclic AMP accumulation and downstream signaling in pancreatic beta cells, adipose tissue and the central nervous system. Because the two pathways overlap only partially, the combined effect on insulin secretion, glucagon suppression and appetite signaling differs from that of selective single-receptor compounds. Affinity is not equal across the two targets, and the clinical meaning of that imbalance remains an area of active study.
Clinical research programs have evaluated tirzepatide in adults with type 2 diabetes and in adults with obesity or excess weight. Trials generally reported reductions in glycated hemoglobin and body weight across treatment periods of several months. Since these studies enrolled defined populations under controlled conditions, the findings describe group averages rather than individual outcomes. Open questions include the durability of effects after treatment stops, variation among subgroups, and the long-term consequences of sustained dual receptor stimulation. Published trial summaries should be consulted for exact measurements rather than secondary accounts.
As a peptide, tirzepatide is handled as a lyophilised solid in research settings and as a preserved solution in finished products. Aqueous solubility is pH dependent and reaches a minimum near the isoelectric point, which lies close to pH 5.4. Stock solutions are typically prepared in neutral or slightly basic buffer to limit precipitation. The solid is hygroscopic and should be equilibrated to room temperature before opening so that condensation does not form on the powder surface.
Recommended storage for reference material is a freezer at approximately -20 degrees Celsius, protected from light and moisture. Commercial injectable presentations are stored refrigerated between 2 and 8 degrees Celsius and must not be frozen. Product labelling generally permits a limited period at controlled room temperature once dispensed, with the exact window depending on the presentation. Repeated temperature cycling is avoided because it can promote aggregation or deamidation of the peptide chain.
Identity and purity are assessed by reversed-phase high-performance liquid chromatography, with mass confirmation by electrospray ionisation mass spectrometry. Peptide mapping after enzymatic digestion verifies the primary sequence. Size-exclusion chromatography quantifies aggregates, while circular dichroism provides a secondary-structure fingerprint. Bioanalytical quantification in plasma uses immunoassay or LC-MS/MS. Reported purity for research-grade lots is commonly 95 percent or higher, and residual water content is checked by Karl Fischer titration.

Dual agonism at the GIP and GLP-1 receptors underlies the observed pharmacology. Activation of GLP-1 receptors raises glucose-dependent insulin release, lowers glucagon secretion, slows gastric emptying and reduces appetite. GIP receptor activation contributes additional effects on adipose tissue and on energy balance, and the combined action on appetite appears larger than either pathway alone in animal models. Signalling bias and the relative contribution of each receptor arm to weight-related effects remain areas of active investigation.
Structure-activity work shows that fatty acid length, linker chemistry and the position of acylation all influence albumin affinity and receptor potency. Plasma protein binding exceeds 99 percent, which restricts distribution and slows renal clearance. Degradation proceeds largely through general proteolysis and fatty acid oxidation rather than cytochrome P450 metabolism, so exposure to common oxidative drug interactions is limited. Whether these clearance routes vary meaningfully between individuals is not fully established.
Unlike most amino acids, which are primarily oxidized in the liver, BCAAs are predominantly metabolized in skeletal muscle and other peripheral tissues. Degradation of branched-chain amino acids involves the branched-chain alpha-keto acid dehydrogenase complex (BCKDH). A deficiency of this complex leads to a buildup of the branched-chain amino acids (leucine, isoleucine, and valine) and their toxic by-products in the blood and urine, giving the condition the name maple syrup urine disease. On the other hand, unchecked activity of this complex causes branched-chain keto acid dehydrogenase kinase deficiency. The BCKDH complex converts branched-chain amino acids into acyl-CoA derivatives, which after subsequent reactions are converted either into acetyl-CoA or succinyl-CoA that enter the citric acid cycle. Enzymes involved are branched chain aminotransferase and 3-methyl-2-oxobutanoate dehydrogenase.
In 1900, the German colonial administration of Nauru granted phosphate rights to British businessman John T. Arundel's Pacific Islands Company (PIC). The PIC was replaced by the Pacific Phosphate Company (PPC) in 1902, with German interests holding around one-third of the company's share capital. Production commenced in 1906, largely relying on indentured labour, with Australia and New Zealand as the primary markets. In 1914, following the outbreak of the First World War, the Australian Naval and Military Expeditionary Force occupied Nauru, with an agreement reached whereby the Australian military would assume administrative control of the island and the PPC would continue phosphate operations. Following the end of the war, Nauru was made a League of Nations mandate under the joint trusteeship of Australia, New Zealand and the United Kingdom, with Australia retaining administration of the island. In 1919, the three trustees signed the Nauru Island Agreement, which entitled them to the phosphate of Nauru through the British Phosphate Commissioners. They bought back all the assets of the PPC for more than 3.5 million pounds on 1 July 1920, and started to manage it directly on 1 January 1921, after a six-month transition period of PPC management. Most of PPC's former employees were retained by the BPC.
=== Genetically encoded polymers === Chilkoti is most known for his work on genetically engineered elastin-like polypeptides (ELPs), which exhibit thermally responsive behavior and have been exploited by his group to develop new technologies and materials for protein purification, controlled drug release, and tissue engineering. He has also pioneered the development of high-throughput and scalable methods for the recombinant synthesis of repetitive peptide polymers that are useful for the recombinant synthesis of ELPs and other repetitive polypeptides. He developed a new non-chromatographic method for the purification of proteins. He also pioneered the development of injectable depots of ELPs fused to peptide and protein drugs for sustained—week or longer—release, and ELP nanoparticles loaded with small molecule chemotherapeutics for cancer therapy. His research group also developed a new class of partially ordered polypeptides that undergo a phase transition from a liquid into a highly porous solid network at body temperature.

=== Glutamate and NMDA receptors === Glutamate levels are observed to be elevated during mania. Lithium is thought to provide long-term mood stabilization and have anti-manic properties by modulating glutamate levels. It is proposed that lithium competes with magnesium for binding to NMDA glutamate receptor, increasing the availability of glutamate in post-synaptic neurons, leading to a homeostatic increase in glutamate re-uptake which reduces glutamatergic transmission. The NMDA receptor is also affected by other neurotransmitters such as serotonin and dopamine. Effects observed appear exclusive to lithium and have not been observed by other monovalent ions such as rubidium and cesium.
Melanotan II is a synthetic analogue of the peptide hormone α-melanocyte-stimulating hormone (α-MSH) that stimulates melanogenesis to facilitate tanning. It may also increase sexual arousal. Melanotan II was originally developed as a tanning agent based on an earlier drug called afamelanotide (previously "Melanotan I") which is now approved by the FDA for the treatment of a particular rare genetic form of sun sensitivity. Melanotan II was later investigated as a potential treatment for sexual dysfunction, but development was abandoned in 2000 in order to pursue development of bremelanotide, a chemically similar drug later approved by the FDA for treatment of sexual dysfunction in women. Unlicensed Melanotan II is found on the internet, although health agencies advise against its use due to legality, case reports suggesting significant safety risks, and potential impurities. Melanotan-II may cause reversible darkening of moles and freckles. It is unclear if Melanotan II can increase (or reduce) the risk of developing melanoma, because reports of melanomas associated with its use have coincided with heavy UV exposure and sun bed use. A 2013 scientific review found there was no conclusive evidence it causes melanoma, and a 2021 review concluded "the increased risk of melanoma in Melanotan users, who use it for tanning and exhibit sun-seeking behaviour, can probably be explained by more UV exposure". Side effects may include facial flushing, nausea and erection in males.
in-frame 1. (of a gene or sequence) Read or transcribed in the same reading frame as another gene or sequence; not requiring a shift in reading frame to be intelligible or to result in a functional peptide. 2. (of a mutation) Not causing a frameshift.
=== Background === Early methods of secondary structure prediction, introduced in the 1960s and early 1970s, focused on identifying likely alpha helices and were based mainly on helix-coil transition models. Significantly more accurate predictions that included beta sheets were introduced in the 1970s and relied on statistical assessments based on probability parameters derived from known solved structures. These methods, applied to a single sequence, are typically at most about 60–65% accurate, and often underpredict beta sheets. Since the 1980s, artificial neural networks have been applied to the prediction of protein structures. The evolutionary conservation of secondary structures can be exploited by simultaneously assessing many homologous sequences in a multiple sequence alignment, by calculating the net secondary structure propensity of an aligned column of amino acids. In concert with larger databases of known protein structures and modern machine learning methods such as neural nets and support vector machines, these methods can achieve up to 80% overall accuracy in globular proteins. The theoretical upper limit of accuracy is around 90%, partly due to idiosyncrasies in DSSP assignment near the ends of secondary structures, where local conformations vary under native conditions but may be forced to assume a single conformation in crystals due to packing constraints.

Red blood cells are used to restore oxygen-carrying capacity in people with anaemia due to trauma or other medical problems Whenever a red blood cell transfusion is being considered for a patient, it is good practice to consider not only the haemoglobin level, but also the overall clinical context, patient preferences, and whether there are alternative treatments. If a patient is stable and has a haematinic deficiency, they should be treated for the deficiency (iron deficiency, B12 deficiency, or folate deficiency) rather than being given a red blood cell transfusion. In adults, blood transfusion is typically recommended when hemoglobin levels are below 70 g/L (7 g/dL) in those who have stable vital signs, unless they have anemia due to a haematinic deficiency. Transfusing at a restrictive haemoglobin threshold of between 70 g/L to 80 g/L (7 to 8g/dL) decreased the proportion of people given a red blood cell transfusion by 41% across a broad range of clinical specialties, including those people who are critically ill. There is no evidence that a restrictive transfusion strategy are stronger associated with death or major adverse events (e.g. cardiac events, myocardial infarction, stroke, pneumonia, thromboembolism, infection) compared with a liberal transfusion strategy. There is not enough information in some patient groups to say whether a restrictive or liberal transfusion threshold is better.
=== Sleep sedation === While benzoctamine was made to be an alternative to the benzodiazepine line of anxiolytic drugs, other uses for the drug have been discovered. Due to benzoctamine's ability to tranquilize without causing respiratory depression, scientists are moving forward with studies that test its sedative effects in patients with respiratory failure. In one study that used benzoctamine in a clinical setting, researchers showed that the use of benzoctamine for sedation did not result in changes in forced expiratory volume in one second or carbon dioxide partial pressure (pCO2). This confirmed previous statements that claimed the drug did not cause respiratory failure. The main goal of this clinical study was to confirm the findings of another study that showed benzoctamine did not reduce CO2 responsiveness, but instead increased the ventilatory response to CO2. There are usually many risks associated with using sedatives on patients who are suffering from respiratory failure, which has made it difficult to administer tranquillizing medications in situations when they are desirable. It is not known why this drug is safe and its benzodiazepine cousins are not, but a possible explanation for this phenomenon might come from its similarity in structure to tricyclic antidepressants, which have also been shown to not cause respiratory failure. While further experimentation is necessary, this study points to benzoctamine's possible consideration for sedation in respiratory failure patients.
On 22 April 2003, The Daily Telegraph published news articles and comment describing documents found by its reporter David Blair in the ruins of the Iraqi Foreign Ministry. The documents purported to be records of meetings between Galloway and Iraqi intelligence agents, and they stated that he had received £375,000 per year from the proceeds of the United Nations (UN) Oil-for-Food Programme. Galloway denied the claims and pointed to the nature of the discovery within an unguarded, bombed-out building as being questionable. He successfully sued the newspaper for libel (see below). The Christian Science Monitor also published a story on 25 April 2003, stating that it had documentary evidence that Galloway had received "more than ten million dollars" from the Iraqi government. However, on 20 June 2003, the Monitor reported that its own investigation had concluded that the documents were sophisticated forgeries. In January 2004, it was reported that documents from Iraq's oil ministry showed that Galloway's Mariam Appeal received money from businessmen who had had allegedly illicitly siphoned profits from the UN oil-for-food program. Galloway said that money had been paid into the Mariam Appeal by Iraqi businessmen who had profited from the UN-run programme. He stated he had not benefited personally and that there was nothing illicit about the transaction:

It is a synthetic peptide that activates both the GIP and GLP-1 receptors, making it a dual agonist. Approved products are given by injection rather than by mouth. It is not a small molecule and does not belong to the older sulfonylurea or thiazolidinedione families.
Engaging two receptors recruits signaling pathways that only partly coincide. This can shift the magnitude of effects on insulin release, glucagon levels and appetite relative to selective agents. Whether the pairing delivers benefits beyond a simple sum of the two is still debated in the literature.
The proportional contribution of each receptor to observed clinical effects has not been fully separated. Long-term consequences of continuous dual stimulation are likewise unclear. Investigators continue to probe these points through laboratory and clinical work.
Solid material is normally kept frozen at about -20 degrees Celsius, desiccated and protected from light. Solutions are held cold and used within a defined window because degradation products accumulate over time.
